Bacterial Meningitis Requirement for Students

If you are a new student UNDER the age of 22, including if you are a transfer student or an Intensive English Language Institute (IELI) student, you must show evidence of a bacterial meningitis vaccination (meningococcal meningitis vaccine) dose or booster.

If you are taking classes online only, the vaccination is not required but if you attend any class or portion of a class on campus or live in on-campus housing, you are required to submit evidence of vaccination.

If you are UNDER twenty-two and you are returning to UNT after a break in enrollment for a fall or spring semester, you are also required to provide your vaccination proof.

Acceptable evidence of receiving a vaccination or a booster dose

The meningitis vaccination should be for the meningococcal ACWY strain.

  • The signature or stamp of a physician or their designee, or public health person on a form that shows the month, day, and year the vaccination dose or booster was administered.
  • An official immunization record generated from a state or local health authority
  • An official record received from school officials, including a record from another state

The meningitis item may remain on your MyUNT checklist even if you fall outside the requirements, but you may still continue your enrollment.

Summer 2024

New students, returning students, and continuing students not enrolled for the previous semester and who are UNDER 22 on the first day of class for the session or term they wish to enroll for, must provide proof of bacterial meningitis vaccination, medical exemption, or State of Texas affidavit of conscience objection notarized no more than 90 days prior by 5 p.m. on the deadline below.

You will be allowed to enroll prior to submitting the documents. However, classes will be canceled and a hold added to your account after 5:00 PM on the last day of regular registration if you have not submitted the proper documentation.

You will only be allowed to enroll during late registration if the necessary immunization documents have been uploaded and requirements satisfied.

Fall 2024

New students, returning students, and continuing students not enrolled for the previous semester and who are UNDER 22 on the first day of class for the session or term they wish to enroll for, must provide proof of bacterial meningitis vaccination, medical exemption, or State of Texas affidavit of conscience objection notarized no more than 90 days prior by 5 p.m. on the deadline below.

Frequently Asked Questions

Where can I get my immunization?

Immunizations are available from the UNT Student Health and Wellness Center, your personal physician, or a local Health Department.

The meningitis vaccination should be for the meningococcal ACWY strain. The following is acceptable evidence of vaccination or receiving a booster dose:

  1. The signature or stamp of a physician or their designee, or public health person on a form that shows the month, day, and year the vaccination dose or booster was administered
  2. An official immunization record generated from a state or local health authority
  3. An official record received from school officials, including a record from another state

The vaccine must have been administered within the last five years or at least ten days prior to the first day of the semester.

How Do I Submit My Immunization or Exemption to UNT?

You can upload your meningitis immunization or UNT Meningitis Medical Exemption Form to your MyUNT portal.

How to upload your file

  1. Log into myUNT
  2. Click on the Tasks tile
  3. Click on "Meningitis Documents Needed" in your To Do List to upload your files
  4. Only one upload is allowed. After upload, your submission will be processed in the order it was received.

Can I get an Exemption?

Medical Exemptions

You, your parents, and your guardians have the right to seek an exemption by completing the UNT Meningitis Medical Exemption Form.

The exemption will only be permitted based on evidence that the bacterial meningitis vaccine would be injurious to your health and well-being according to the professional opinion of a physician who is registered and licensed to practice medicine in the United States.


Reasons Of Conscience Exemptions

Exemptions from Immunizations for Reasons of Conscience, such as religious or philosophical belief, can only be requested through the Texas Department of State Health Services. The exception form from DSHS must be signed and notarized no more than 90 days prior to the first day of classes and submitted in-person to the Registrar in order to be valid.
